This isn't about any teacher doing a bad job. An exam has to be gradable on paper, in a fixed amount of time — so it tests what's checkable: syntax, correct output, definitions. The bigger picture — why this code exists, what it connects to, what breaks if it's missing — is much harder to put on an answer sheet. So it quietly gets left out, for everyone, everywhere.
Picture a finished bridge. Everyone who drives across it sees the same thing — a road, some cables, cars moving smoothly. An engineer looks at that same bridge and also sees the forces holding it up, the materials chosen for a reason, why it was built exactly there. Most programming classes only ever show a child the finished bridge — never what's holding it up.
